State Tariffs and Catalog Schedules

MCImetro Access Transmission Services LLC, MCI Communications Services, Inc., TTI National and Telecom*USA no longer file tariffs or price lists with the Public Utilities Commission of Ohio (PUCO) for intrastate retail interexchange services and for non-residential Tier 2 local services as defined by the PUCO, except that tariff provisions are still applicable to PIC (Primary Interexchange Carrier) change charges, Alternative Operator Services, late payment and bad check charges specific to Ohio intrastate services, per call blocking and per line blocking, N11 services, and TRS (Telephone Relay Services). The complete terms and conditions that govern these non-tariffed services in Ohio are:

• For intrastate services that are subject to the Service Publication and Price Guide, see the customer's written service agreement and the online Service Publication and Price Guide (at www.verizonbusiness.com/guide), and the tariffs below for those services which are still subject to tariffs.
• For intrastate services that are subject to the MCI, TTI National, or Telecom*USA General Services Agreement for Residential Customers, see the General Service Agreement, any optional calling plan, promotion, and/or authorized written communications received from MCI Communications Services, Inc., TTI National, or Telecom*USA, Inc. and the Catalog Schedules set forth below, and the tariffs below for those services which are still subject to tariffs.
• For intrastate services that are subject to the MCI General Services Agreement for Small Business Customers, see the General Service Agreement, any optional calling plan, promotion, and/or authorized written communications received from MCI Communications Services, Inc. or Telecom*USA, Inc. and the Catalog Schedules set forth below, and the tariffs below for those services which are still subject to tariffs.
